Health Plans, Rate Charts and Benefits Guide

The City offers the following Health Insurance plans to eligible employees:

  • UnitedHealthcare (UHC) Choice EPO Plan
  • UnitedHealthcare (UHC) Choice PPO Plus Plan
  • UnitedHealthcare (UHC) High Deductible Health Plan

Enrollment / Self Service

Employees are responsible for keeping their enrollment status, including births and marriages, current through the City's Self Service program at www.milwaukee.gov/selfservice. All employees must have their Employee ID number (6-digits) and a password to access self service. To request or reset a password go to www.milwaukee.gov/rits.

Employees who do not enroll in the City's Health plan must complete the Health Waiver Form below for Affordable Care Act (ACA) reporting purposes.  See the Health Enrollment Forms section below for the Health Waiver Form.

Active City of Milwaukee employees should return the completed form to DER Employee Benefits, City Hall, Room 706 or derbenefits@milwaukee.gov.  Retirees should return their forms to Employees' Retirement System and agency employees (HACM, WCD, and MEDC) should return the completed form to their department payroll.

Why is UHC Calling?

It may seem unusual, but the next time your phone’s caller ID reads “UnitedHealthcare,” there’s a good chance it’s an offer to connect you with information to help you manage or improve your health. These are not sales calls, rather, these calls are used to extend invitations to members including wellness coaching, disease management and pregnancy resources.  Watch this short video to learn more.

For Medical and Pharmacy Questions

Employees that have medical or pharmacy benefit questions regarding unpaid bills or problems with service should call the health or pharmacy plan first.  DO NOT call Employee Relations until the appropriate plan has been contacted.  If the situation cannot be resolved through the plan provider, Employee Benefits will attempt to assist with resolving the problem.  Remember to document all phone conversations with dates, times and names and save any email correspondence.  Employee Benefits will ask members for this information to try to resolve the issue.


All City members have one card for health and prescription drugs. The UHC information is at the top left of the card and the OptumRx information is on the bottom right side. Members with questions about medical or prescription coverage should call the number on the back of the card, 1-800-841-4901.

Enrollment Status | Self Service

All employees must have their login ID and a password to access Employee Self Service. To request or reset a password go to email HR at askHR.com.

Employees who do not enroll in the Housing Authority’s health plan must complete the Health Waiver Form below for Affordable Care Act (ACA) reporting purposes. Please return the Waiver Form to HR at askHR.com
