Americans with Disabilities
It is the policy of HACM to provide equal employment opportunities for all employees and applicants, as required by applicable state and federal law. It is the Organization’s policy, in compliance with the Americans with Disabilities Act (ADA) and the Wisconsin Fair Employment Act (WFEA), to reasonably accommodate a qualified individual with a disability where such accommodation does not create an undue hardship on the operation of HACM’s business. Harassment of an employee because of a real or perceived disability, or record of a disability, will not be tolerated.
If an employee wishes to request a reasonable accommodation, the employee should contact the Human Resource Office, who will then engage in an interactive process with the employee to identify reasonable accommodations that can be made to allow the employee do perform his or her job. The employee and the employee’s medical provider must provide relevant information, including but not limited to the following: :
1. Identifying the disability and/or medical conditions and restrictions
2. Identifying the reasonable accommodations being requested
Both the employee and the employee’s medical provider must sign the request certifying the statements are correct and accurate.
It is also the desire of HACM to make its facilities accessible to customers and employees with disabilities. If an employee is aware of areas or services in and around HACM facilities that are not accessible to persons with disabilities, please contact the Manager or the Human Resource Department. The Human Resource Office or appropriate supervisor will thereafter engage in an interactive process with the employee to determine reasonable accommodations, if any.
